Finding a provider with Aetna
For employees enrolled in Aetna medical and dental, we've made it easy to search your Aetna plan name via Aetna's website and find a provider that is In-Network.
AETNA MEDICAL
Find a doctor before you enroll
Step 1: Go to Aetna's Find a Doctor tool
Step 2: Enter your home zip code in the Continue as a guest box and select Search.
Step 3: Search for the name of the plan you are looking at enrolling in.
Pro Tip: You can find the name of the plan you are looking at either on the plan selection screen under Provider Plan Key or on the downloadable Plan Details.
Step 4: Search for a provider to find if they are in-network.
Find a Doctor after you have enrolled
Step 1: Log into Aetna's member portal.
Step 2: Go to Find Care & Pricing
Step 3: Search for a provider or procedure.
AETNA DENTAL
Find a Dentist before you enroll
The process for finding a dentist before you enroll is the same as the above process for finding a doctor before you enroll.
Pro Tip: You can find the name of the plans you are looking to enroll in on the Provider Plan Key on the plan selection page.
Find a Dentist after you've enrolled

Finding a provider with MetLife
For employees enrolled in MetLife dental, we've made it easy to search your MetLife plan name via MetLife's website and find a provider that is In-Network.

Step 3: Choose your network
Pro Tip: The network is PDP plus for all MetLife plans.
Step 4: Enter your zip code.
Step 5: Enter the name of the provider you are searching for.
Finding a provider with UnitedHealthcare
For employees enrolled in UnitedHealthcare, we've made it easy to search your UHC plan name via UHC's website and find a provider that is In-Network.

Find a doctor before you are enrolled
Step 1: Visit UHC's find a doctor portal.
Step 2: Select Start your search on the Search Our General Provider List card.
Step 3: Select what type of provider you are searching for. This will be either a medical or behavioral health provider.
Step 4: Select Employer and Individual Plans.
Step 5: Select either UHC Member (if you are already enrolled to sign in) or Shop Around for before you enroll.
Step 6: Find the plan you are looking for. All of our plans are Choice Plus.
Step 7: Search for the doctor or service you are looking for.

Finding a provider with Kaiser Permanente
For employees enrolled in Kaiser Permanente, we've made it easy to search your Kaiser Permanente plan name via KP's website and find a provider that is In-Network.

How do I know which plan network I'm on once I am enrolled?
You can view your plan network directly from the insurance cards in the Benefits Center in your Justworks account.
Please keep in mind that the network names that the Find-A-Doc tool uses may differ slightly from what you see in your Justworks account, but you can reach out to Aetna directly to confirm.

Anthem (Blue Cross Blue Shield)
Visit the Anthem (Blue Cross Blue Shield) Find Care page. From here, you will see options to “Log in for Personalized Search” under your Anthem account. This will allow you to find doctors, hospitals, and more in your plan's network. You can also get detailed estimates for procedures or services (not available with some plans). If you have your member ID, you can use the “Use Member ID for Basic Search” function to find doctors, hospitals, and more near you without needing to log into your Anthem account. If prompted to enter a network name, you can find this information directly in the Justworks enrollment flow or within the downloadable plan details PDF from Justworks. There will also be an option to use a “Basic search as a guest” to select a plan and find out if a doctor, hospital, or other care provider is in-network.
Blue Cross Blue Shield of Texas
Visit the Blue Cross Blue Shield of Texas Find a Doctor or Hospital page. If you have created your account, you will see the “Member Login” option. From here, you can log in to your account to get the most accurate, personalized search results based on your plan. If prompted to enter a network name, you can find this information directly in the Justworks enrollment flow or within the downloadable plan details PDF from Justworks. You’ll see details that may help you lower healthcare costs. This tool will also display doctors in your plan network, doctor reviews and ratings, and cost estimates. If you have not yet created your account, you can use the “Guest Search” feature. Even if you’re not a member, you can search for doctors, hospitals, and other providers.
Empire (Blue Cross Blue Shield)
Visit the Empire (Blue Cross Blue Shield) Find Care page. From here, you will see options to “Log in for Personalized Search” under your Empire account. From here, you can log in to your account to get the most accurate, personalized search results based on your plan. Select the type of coverage as “Employer-Provided” and the type of provider as “Medical.” Choose the network as “Statewide/National PPO (PPO)” or “Statewide HMO (HMO),” enter your zip code, and search by doctor name, type, etc.

Visit the Horizon BlueCross BlueShield Doctor Finder page. If you have created your account, you will see the “Member Login” option. From here, you can log in to your account to get the most accurate, personalized search results based on your plan. If prompted to enter a network name, you can find this information directly in the Justworks enrollment flow or within the downloadable plan details PDF from Justworks.
