How To File a Claim for MetLife Benefits with Justworks

There are a few ways you can choose to file a claim for your Short Term Disability (STD), Long Term Disability (LTD), New York Paid Family Leave (NY PFL) or New York Disability (NY DBL) polices provided by MetLife through Justworks. Keep reading to learn the different options below.

 

Metlife Member ID

When filing a claim for Short Term Disability (STD), Long Term Disability (LTD), you can opt to use your Metlife Member ID instead of the Social Security Number on the claims form. You will input the Member ID in the “Employee ID number (if applicable)” field. If using the Member ID number for STD or LTD claims, you can leave the “Social Security Number” field blank. Please note, this option is only available for STD and LTD claims, for all other claims you will continue to provide your SSN.

 

Filing A Claim Online

MetLife’s “MyBenefits” portal allows you to file a claim online, view the status of a claim, and also set up direct deposit information all in one place. 

To get started:

  1. Navigate to the “Ancillary Benefits” tab of your Benefit Center in Justworks to see what MetLife products you are currently enrolled in through Justworks. 
  2. Click on the “Get Started” button to see some helpful information about your policy and everything you need to get ready to file your claim with MetLife.Screenshot 2023-09-18 at 4.46.27 PM.png
  3. Click on the “File online” button to be brought to MetLife’s "MyBenefits" portal.
    1. When prompted for the name of your organization enter Justworks Employment Group, LLCScreenshot 2023-09-18 at 4.28.08 PM.png
  4. If you have already created an account you can select “Login” to enter the portal
  5. If this is your first time filing a claim, select “Register
    1. Complete the account creation process using the information as it appears in your Justworks profile
  6. Once in the MyBenefits portal, navigate to the “Claims Center” and select “I want to file a claim” in order to begin the claims filing process.

 

Filing your Claim on the Phone

Starting your claim over the phone is easy by calling MetLife’s personalized 1800 number for Justworks members 1-833-771-1424.

You can also call this number for help understanding your MetLife Policies .

 

Filing your Claim via Fax or Mail

If you’d rather file a claim by faxing or mailing in a paper claim form as you always have, you now have access to your claims form in your Justworks profile

  1. Navigate to the “Ancillary Benefits” tab of your Benefit Center in Justworks to see what MetLife products you are currently enrolled in through Justworks. 
  2. Click on the “Get Started” button to see some helpful information about your policy.
  3. Click “Download Claims form” for a copy of your policy’s claim form. You can submit your completed claims form to MetLife via fax or mail to the number or address below:
    1. FAX: 1-800-230-9531 (preferred method)
